Substance Use Disorder (SUD) Case Management provides structured, ongoing support to individuals in recovery from alcohol and drug dependency. Our certified case managers work alongside clients to assess their needs, create individualized recovery plans, and connect them with the full range of services necessary to achieve and maintain long-term sobriety.

SUD Case Management addresses the interconnected challenges that often accompany substance use — including housing instability, unemployment, trauma, and co-occurring mental health conditions — with a coordinated, whole-person approach that increases the likelihood of lasting recovery.
